9 Part-time 1 day ago Full Job Description JOB Provides care coordination and medical nursing case management services to program clients.
Works with the Outreach and Community Development Specialist to establish partnerships and referral networks, including linkages between the program and health care providers.
Participates as a member of the Community Action Network and communicates perinatal health system issues to the CAN for consideration and action.
EXAMPLE OF DUTIES The NCC serves as the primary case manager or consultant for high-risk women identified through the risk assessment tool as having chronic disease (i.
e.
diabetes, hypertension, etc.
), behavioral or mental health issues, and significant reproductive health risks, and will perform all duties listed under CHW description in addition to nursing specific assessments, education and activities defined by the Loving Steps program.
Provides training and education to the Loving Steps team on issues related to health, such as reproductive methods and injury prevention; educate and guide CHWs on when and how to seek nurse consultation related to a client's case.
The NCC will have primary responsibility for care coordination with the clients' pregnancy or primary care medical home as applicable.
Duties include identifying and introducing him/herself to the client's primary care provider (PCP); explaining the program, her role, and the purpose of the contact; informing them of their clients enrollment in Loving Steps and communicating risk assessment and care plan results to the PCP as appropriate; contacting the PCP as needed to discuss health care goals and opportunities for mutually reinforcing messages and health education and promotion opportunities.
Links, or assists CHW to link, clients identified as at risk using screening tools to appropriate service in the community.
Responsible for planning, teaching and/or identifying external experts to provide group client/staff education or training related to health and wellness.
Actively participates as a member of the Community Action Network.
Works with the Program Coordinator to complete monthly and quarterly reports to the State Project Director and attends quarterly grantee meetings and special meetings with other key staff.
MINIMUM QUALIFICATIONS Minimum Education and
Experience:
Registered Nurse or LPN, preferably with perinatal nursing experience.
A solid understanding and experience in supervising diverse staff with humility as well as providing support to staff in stressful work environmentExperience in home visiting preferred.
Licenses and/or Certifications:
Infant mental health endorsement preferredA valid Virginia driver's permit, a workable automobile with appropriate insurance coverage.
Knowledge:
Knowledge of family dynamics, child development, and community resources.
Knowledge of issues related to maternal-child health, infant mental healthily, and child abuse and neglect.
Skills:
Demonstrated skill in interviewing and communicating with clients and professionals in the community.
Basic computer and word processing knowledge.
Critical thinking, complex reasoning, and analytical skills.
Basic math.
Organization and time-management skills.
Problem-solving, customer service and communication (oral, written, and listening) skills.
Skilled in teaching parents to interact and meet the needs of their children, training others in agency on how to use curriculum, interventions, etc.
Abilities:
Ability to work effectively with people of diverse cultures, humility in successfully working with different background.
Ability to plan effectively for client services.
Willingness to engage in building reflective capacity (capacity for introspection, communicating awareness of self in relation to others, recognizing value of supervision).
Demonstrates accuracy and an understanding of confidentiality of client records and need for follow up on issues with other community professionals and clients.
Must pass a criminal and Social Services background check.
SUPPLEMENTAL INFORMATION Work Environment:
Work is generally performed in a moderately noisy location (e.
g.
business office, family homes, light traffic), requiring regular use of motor vehicles, mechanical, and/or technical equipment, and observing general surroundings and activities.
Worker maybe exposed to extreme noise, irate clients, and to potentially dangerous people or situations.
Work regularly requires exposure to outdoor weather conditions, frequently requires exposure to blood borne pathogens which may require the use of specialized personal protective equipment, and occasionally requires exposure to fumes, airborne particles, and/or toxic or caustic chemicals.
Essential Physical Activities:
Stooping, crouching, walking, pulling, lifting, grasping, hearing, seeing up close, seeing far away, kneeling, reaching, pushing, talking, standing, finger movement, repetitive motions, and depth perception.
Typical weight handled up to 25 lbs.
