Clinical Pharmacy Manager

Responsibilities
The Pharmacy Clinical Manager is responsible for designing, implementing and managing the pharmacy clinical programs. This includes overseeing the preparation for the Pharmacy and Therapeutics Committee, formulary management, policy and procedure development and education across the healthcare spectrum. The Pharmacy Clinical Manager serves in a supervisory capacity within the pharmacy department; therefore, will assist with managing the pharmacy when the Pharmacy Director is unavailable.
€ Interpret orders and transcribes or verifies them accurately and efficiently into the pharmacy computer system
€ Accurately compounds and dispenses pharmaceuticals
€ Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose
€ Provide clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed.
€ Sustain the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and promotes rational drug therapy selection
€ Support participation in daily multidisciplinary and bedside rounds (may need to attend)
€ Identify, design and analyze drug utilization reviews to promote rational drug therapy and determines action plan based on results
€ Provide accurate and timely medication information to healthcare staff and patients
€ Develop, manage and mentor pharmacy clinical programs such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as determined appropriate based on patient population and literature guidelines.
€ Develop, manage and mentor antimicrobial management program (AMP).
€ Document all clinical activities and interventions accurately and completely. Responsible for mentoring and education as well as ongoing analysis of interventions to insure accurate and appropriate documentation and provides summary reports
€ Support and participate in the goals of the organization's quality improvement, risk management and patient safety program to include but not limited to National Patient Safety Goals, Core Measures and HCAHPS medication related initiatives.
€ Responsible for design, analysis and reporting of adverse drug reactions, medication errors and pharmacy quality initiatives. Mentors others in their roles for these programs
€ Develop and actively participate in training and mentorship programs for pharmacists, pharmacy students, residents, technicians, nurses and physicians. Responsible for assessment of participants in these programs. Provides a minimum of two (2) inservices per year on rational medication therapy, disease management, and pharmaceutical care effectively to health care professionals.
€ Develop and document staff competency on an ongoing basis and develops improvement plans when deficiencies in competencies are identified which includes the review of the performance and documentation of clinical activities of all pharmacists including documentation of the financial impact of clinical activities.
€ Primary preceptor for pharmacy students and/or residents as it relates to clinical pharmacy services. Responsible for designing and managing program and mentoring student/resident as well as other preceptors
€ Maintain professional competency sufficient to meet current and future needs of the facility which includes reviewing and evaluating medical literature for studies supporting the pharmaceutical care and pharmacoeconomic programs.
€ Develop and manage specialty pharmacy practice programs as related to clinical pharmacy services
€ Effectively and proactively intervene in conflicts between pharmacy and medical staff regarding drug therapy issue resolutions.
€ Maintain appropriate controlled substance records.
€ Comply with all regulatory standards. Work with the pharmacy director to identify and develop processes to meet or exceed all regulatory standards. Participate in quality and regulatory reviews and audits
€ Prioritize work and leads others to comply with facility medication turn-around time policy and procedure. Identify areas for improved efficiency
€ Supervise and assist pharmacy technicians and other support staff effectively
€ Work with the director to establish pharmacy goals and objectives and responsible for implementation and monitoring
€ Participate with various nursing and medical staff committees as necessary
€ Participate with various hospital committees which may include the disaster team, code team, etc.
€ Responsible in conjunction with director of pharmacy for staff performance evaluations
€ Act as manager in charge as assigned
€ Responsible for development and review of physician order sets and other evidenced based medication use guidelines
€ Responsible for routine publication of the pharmacy newsletter and/or other forms of written and electronic communication
€ Responsible for drug class reviews and drug monographs
€ Responsible for managing and monitoring the drug formulary
€ Responsible for the development, implementation, and monitoring of pharmacy clinical initiatives
€ Responsible for the development and review of pharmacy policies and procedures as appropriate for clinical services
€ Practice and adhere to the "" Code of Conduct"" philosophy and ""Mission and Value Statement""
€ Collaborates with laboratory in the development of an antibiogram annually.
€ Participates in community educational programs (e.g. , diabetes classes, health fairs).
Qualifications:
Education & Experience:
€ B.S. or Pharm D. from an ACPE-accredited School of Pharmacy Required
€ 5 years of experience in hospital pharmacy preferred
€ 3 years of experience in management Preferred Or equivalent combination of education and/or experience
Licenses, Certifications, & Training:
€ Pharmacist- must be licensed in the state of Virginia or able to obtain licensure within 30 days of hire
€ Board Certification in Pharmacotherapy, or other appropriate certification preferred

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
